Monster Blood is a C-rank passive item in Enter the Gungeon. If you have Monster Blood in your inventory, you will be immune to poison and gain a heart container when you pick up the item in the Gungeon. You can also spawn a pool of poison creep whenever you take damage while this item is in your inventory.
Is the Monster Blood a good item in Enter The Gungeon?
Monster Blood is a very good item in Enter the Gungeon. It provides poison immunity and an extra heart container upon pickup. The ability to spawn a pool of poison creep is nice to have as well.
On its own, Monster Blood is already a pretty good passive item to have in the Gungeon, but when paired with any of its six Synergies with other items/guns in the Gungeon, Monster Blood can bring out even more from guns such as the Shotgrub and Plague Pistol.

Monster Blood shares six Synergies with other items/guns in Enter the Gungeon – Deadly Distraction, Deadlier Distraction, Monster Grub, Natural Selection, Pandemic Pistol, and Venom Veins.
Deadly Distraction
The Deadly Distraction Synergy between the Monster Blood and the Decoy will cause the latter to create a large poison pool whenever your Gungeoneer is attacked.
Deadlier Distraction
If you have the Monster Blood and the Explosive Decoy in your inventory, you will be able to spawn an even bigger poison pool than the one found in the Deadly Distraction Synergy.
Monster Grub
If you have the Shotgrub alongside the Monster Blood in your inventory, the Monster Grub Synergy will become active.
Monster Grub will give the Shotgrub’s bullets homing capability, giving it the ability to home in on targets around the Gungeon.
Natural Selection
Natural Selection is a Synergy between the Monster Blood and the Evolver. The Natural Selection Synergy will cause the Evolver to evolve after killing just 3 unique enemies instead of the usual 5.
Pandemic Pistol
The Monster Blood and the Plague Pistol can combine to activate the Pandemic Pistol Synergy. Pandemic Pistol will turn the Plague Pistol red and give it increased ammo capacity. The Plague Pistol will also gain linked bullets that hurt enemies that come into contact with it.

If you have the Monster Blood and the Mutation, the Venom Veins Synergy between these two items will cause the Mutation to inflict poison on enemies damaged by the Gun.

Monster Blood is a passive item in Enter the Gungeon, which means you only need to have the item in your inventory to use its effects. For the most part, Monster Blood only provides passive bonuses such as immunity to poison and an extra heart container. You will also spawn a pool of poison creep whenever you take damage.
The Monster Blood’s passive effects are nice to have, but its Synergies with other items/guns are arguably what makes it an even better item that can bring out more from many guns/items in the Gungeon.
