Upon pickup, the Old Crest will give you a piece of armor instantly, but this piece of armor will override any existing armor that you have equipped. This means that the Old Crest will function as your primary armor as soon as you pick it up, making the Old Crest very vulnerable to incoming fire.
If you want to unlock the Abbey of the True Gun, you need to avoid taking damage as best you can as soon as you pick up the Old Crest. You can unlock the Abbey of the True Gun as soon as you place the Old Crest in the altar in the Gungeon proper.
You can protect the Old Crest if you have a Full Metal Jacket since the Full Metal Jacket will automatically trigger one of your Blanks before you take damage in the Gungeon.
The Old Crest cannot be dropped and it cannot be stolen by the Resourceful Rat.
